A Power of Attorney is a document that is prepared for you and signed by you that assigns someone to be your "Agent" to assist in making decisions for you while you are alive, but are unable to make those decisions.
In Illinois there are two types of Powers of Attorney: Power of Attorney for Property and Power of Attorney for Health Care. You will need both of these Powers of Attorney to be properly protected.

This Power of Attorney allows your Agent to make decisions regarding your Health Care. This allows your Agent to have the same access to your medical records that you do, it allows them to consult with your doctor or medical provider regarding your medical care. This document also provides your wishes for End of Life Decisions and gives your Agent authority in fulfilling those wishes.
